Step 1: Fill out the General Information form below to begin the evaluation process. This information allows our staff to verify insurance benefits as well as general information regarding the client being evaluated.

Step 2: Please contact the physician's office to request a referral for specific therapy services for the client whom is to be evaluated. This can be faxed to our office at 855-246-2163.

Step 3: You will receive a phone call from a Brains In Motion staff member to discuss insurance benefits and if desired, to schedule the client's evaluation at that time.

Step 4: Please fill out the emailed forms and submit them to Brains In Motion via email before the client's initial evaluation.
